PERSONAL INFORMATION

EACH PARTICIPANT MUST REGISTER SEPARATELY

Emergency Contact Information

NO PETS, FIREARMS, DRUGS, USE OF TOBACCO OR ALCOHOL PERMITTED ON CRI PROPERTY

I understand that I am registering for an event that includes my participation in more than the speaker content and classroom instruction. This includes, daily worship and prayer, and possibly small group, base life and base service projects. I understand that our events are not a conference but an immersive experience and I agree to fold into the flow of CRI Mission Base life for the time I am there.

Each attendee must register.
There will be no refund for cancellations unless you are not approved for participation in some specific events. Medical emergency cancellations will be taken into consideration on a case by case basis with a letter from an M.D. Submit cancellation requests to: [email protected]

In the unfortunate case of an event cancellation by Crisis Response International, CRI will refund your registration fees at 100%, but will not assume responsibility for refunding personal charges incurred (ex: travel costs, etc.).

DISMISSAL WAIVER

Voluntary Resignation or Withdrawal
There may be situations where both CRI and the individual consider voluntary resignation or withdrawal from a program a reasonable option based on the circumstances.

Entry or Return to Base Property
All responders, staff, and volunteers are required to leave the mission or off-site property immediately or at the earliest possible opportunity after withdrawal. Terms of entry or return to mission or off-site property may be dictated by CRI as part of the withdrawal process.

Dismissal
There may be situations where CRI considers the dismissal of the individual from all serving, ministry, trips, training or school as a necessary step for reasons including but not limited to the protection of the mission campus and CRI.

Entry or Return to Campus of Off-Site Property
Upon dismissal, all responder, staff, or volunteers are required to leave the campus property immediately, and may only enter the campus with permission from the Director or his designee and will require an escort while on any campus property.

Trespass
Because the CRI Mission is private property, all responders, staff, and volunteers that have been dismissed from any training, serving or ministry are requested not to enter campus property unless specifically invited by mission campus or CRI leadership.

While the campus does host activities that are open to the general public, any responders, staff, and volunteers that have been dismissed (or withdrawn) from any training, ministry or service must get written permission from the CRI Director or his designee. The mission may detail specific requirements and limitations of the “persona non grata” while on campus property.

Refunds
There will not be any refund for dismissal, self-termination or withdrawal.

Costs Associated With Termination/Withdrawal/Resignation
CRI is NOT responsible for all subsequent costs and travel arrangements associated with departure.

I understand that the activities may include, but are not limited to: transportation in commercial, private or CRI-owned vehicles; participating in various types of activities using a variety of modalities. The World Health Organization has declared the novel Coronavirus (COVID-19) a worldwide pandemic. COVID-19 is reported to be extremely contagious. The state of medical knowledge is evolving, but the virus is believed to spread from person-to-person contact and/or by contact with contaminated surfaces and objects, and even possibly in the air. People reportedly can be infected and show no symptoms and therefore spread the disease. Evidence has shown that COVID-19 can cause serious and potentially life-threatening illnesses and even death. CRI cannot prevent you or your children from becoming exposed to, contracting, or spreading COVID-19 while attending CRI events. Therefore, if you choose to participate in any CRI activity, you may be exposing yourself to and/or increasing your risk of contracting or spreading COVID-19.

Assumption of Risk
With full knowledge of the risks involved, I hereby choose to accept the risk of contracting COVID-19 for myself and/or my children in order to attend any CRI activity. These services are of such value to me, and/or to my children, that I accept the risk of being exposed to, contracting, and/or spreading COVID-19 in order to attend a CRI event in person.
I hereby release, waive, discharge CRI, its board, officers, independent contractors, affiliates, employees, representatives, successors, and assigns from any and all liabilities, claims, demands, actions, and causes of action whatsoever, directly or indirectly arising out of or related to any loss, damage, injury, or death, that may be sustained by me related to COVID-19 while participating in any activity while in, on, or around the premises or while using the facilities that may lead to unintentional exposure or harm due to COVID-19.

I agree to indemnify, defend, and hold harmless CRI from and against any and all costs, expenses, damages, lawsuits, and/or liabilities or claims arising whether directly or indirectly from or related to any and all claims made by or against any of the released party due to injury, loss, or death from or related to COVID-19.

By signing below I acknowledge that I have read the foregoing Liability Release Waiver and understand its contents; that I am at least eighteen (18) years old and fully competent to give my consent; That I have been sufficiently informed of the risks involved and give my voluntary consent in signing it as my own free act and deed; that I give my voluntary consent in signing this Liability Release Waiver as my own free act and deed with full intention to be bound by the same.
