Puerto Rico Trip Info

Thank you for choosing to register for this life-changing deployment to Puerto Rico. A few quick pieces of information to read through before you complete your registration.

Trip Dates: 

Wave 18 - July 20th through July 29th

Total Trip Cost per Wave: $650 (does not include your airfare/transportation costs to and from SJU airport in San Juan, Puerto Rico.)

Flight Arrangements: DO NOT BOOK YOUR FLIGHTS UNTIL YOU RECEIVE APPROVAL FROM CRI DEPLOYMENT DEPARTMENT!!

You are responsible for purchasing your airfare to and from SJU airport in San Juan, Puerto Rico. 

   + For Wave 18, you must arrive on July 20th and with a departure of July 29th, 2019. 

Flight arrivals need to be between 8:00 am and 8:00 pm. Please plan your arriving flights accordingly.  

Return travel should be arranged with your flight departing between the hours of 8:00 am and 8:00  pm on the last day of your selected trip. If you have trouble finding flights that fit into this time frame please contact us before booking your tickets to make special arrangements.


Personal Fundraising Through CRI
 You may also fundraise for your deployment through CRI. Simply click this link and fill out the form. Follow the few easy steps and you will create a personal fundraising campaign that can be used through text, email, and social media. 100% of what comes in (less credit card fees) will be applied to your deployment fee. DO NOT BEGIN FUNDRAISING FOR THIS TRIP UNLESS YOUR DEPOSIT IS PAID AND YOU HAVE BEEN APPROVED AND CONTACTED BY THE DEPLOYMENT DEPARTMENT.

Wave 18 - The balance of your deployment fee is due by July 13th, 2019.

Release and Waiver of Liability

I, the Participant, desire to volunteer with Crisis Response International to provide emergency disaster relief services and engage in the activities related to offering these services. I understand that the activities may include, but are not limited to, travel to disaster sites in the United States and Internationally; transportation in commercial, private or CRI-owned vehicles; moving and lifting heavy objects; cooking and serving food; and working and inhabiting environments that may be without power, sanitation, or are otherwise damaged by the disaster event. I hereby freely and voluntarily, without duress, execute the Release under the following terms:

I, the Participant, release and forever discharge and hold harmless Crisis Response International from any claim or liability that I, the participant, may have against Crisis Response International with respect to any bodily injury, personal injury, illness, death or property damage that may result from my participation in a disaster relief operation. I also understand that Crisis Response International does not assume any responsibility or obligation to provide financial or other assistance, including, but not limited to medical, health, or disability insurance, in the event of injury, illness, death or property damage (see insurance requirements below).

Crisis Response International does not carry or maintain, and expressly disclaims responsibility for providing any health, medical or disability insurance coverage for the Participant. EACH PARTICIPANT IS EXPECTED AND ENCOURAGED TO CARRY PERSONAL LIABILITY OR HEALTH INSURANCE PRIOR TO REGISTERING AS A CRISIS RESPONSE INTERNATIONAL DISASTER WORKER.

Except as otherwise agreed to by Crisis Response International in writing, I hereby release and forever discharge Crisis Response International from any claim whatsoever which arises or may hereafter arise on account of any first-aid treatment or other medical services rendered in connection with an emergency during my time with Crisis Response International.

I understand that my time with Crisis Response International may include activities that may be hazardous to me, including, but not limited to, cooking and food preparation activities, loading and unloading of heavy equipment and materials, transportation to and from the disaster site, and working in locations damaged by the effects of a disaster. I recognize and understand that my time with Crisis Response International may, in some situations, involve inherently dangerous activities. I hereby expressly and specifically assume the risk of injury or harm in these activities and release Crisis Response International from all liability for injury, illness, death or property damages resulting from the activities of my time with Crisis Response International.

I grant and convey unto Crisis Response International all rights, title and interest in any and all photographic images and video or audio recordings made by Crisis Response International during my work for Crisis Response International, including, but not limited to, any royalties, proceeds or other benefits derived from such photographs or recordings.

I understand that it is my desire to further the work of Crisis Response International by performing services as a Volunteer, specifically as a Volunteer in Emergency Disaster Services. I undertake to perform said services as a Volunteer without compensation and that, in performing said services, I acknowledge that I am not acting as an employee of Crisis Response International.

By checking this box, I represent that I am at least 18 years of age and otherwise competent to sign this Release of Liability agreement. This Release of Liability & Indemnification Agreement shall be binding to the fullest extent permitted by law. If any part of this release is deemed to be unenforceable, the remaining terms shall be enforceable as a contract between parties. This release shall be binding upon my assignees, subrogors, heirs, next of kin, executors and personal representatives.

By checking this box, I acknowledge that I may be found by a court of law to have waived my right to maintain a lawsuit against Crisis Response International, including claims that Crisis Response International has committed negligent acts or omissions. I have had a sufficient opportunity to read this entire Release and Liability section. I have read and understood it, and I agree to be bound by its terms.

I, the participant or legal guardian, whose name is listed above, agree to all the terms listed above that I have read and checked the appropriate box, signifying my agreement with each written statement.
