This application will require approximately 30 minutes to finish and submit. You will NOT be able to save and come back to your information. Please plan your process appropriately so that you have all of the necessary information and do not lose your work. If you lose your application information, you will need to start over again.

At the end of the application you will be required to submit online payment information for your NON-REFUNDABLE application fee of $35. Please ensure you have your payment information available.

After you submit your application you will receive an email with a link to reference forms. Forward the worship leader reference link to a someone who has given instruction or oversight to your worship leading (not a friend or relative who likes your style). This is in addition to your two references mentioned above.

OFFICIAL AGREEMENT AND REQUIRED SIGNATURE

I understand that payment of the required school tuition fees must be made in U.S. currency prior to or upon the final due date. Furthermore, I agree to meet in a timely manner, prior to the completion of school, all expenses incurred during my involvement with Crisis Response International.

I understand that CRI reserves the right to deny admission based on information provided in this application, derived through contact with the references I provide, and/or through a subsequent background check. If I am accepted, I will abide by the spirit, rules and schedule of the school, understanding that non-compliance could result in my expulsion without a refund. We also encourage you to invite others into your journey to pray with you and believe your needs will be met. A $35 non-refundable application fee is due when submitting application. ONLY PAY $35 NOW after filling out the application.
A $500 deposit is due upon acceptance and is required to hold your space in the school.
